Required before pickup

Participant Safety Pickup Form

Everyone picking up a child must complete this form and show both a photo ID and the confirmation email to the participant's instructor.

Important reminders

  • If the pickup person is not listed as an emergency contact or has a different last name than the child, staff will confirm permission before release.
  • To avoid delays, parents should notify on-site staff in advance if someone else is picking up.
  • You must inform us on day one of anyone not authorized to pick up your child. If that person arrives, we will contact the authorities.

  • Late pickup fee: $1/minute after the posted end time.
  • If you'll be late, text your child's instructor — or call us at (314) 252-8344.
